The following packet includes the paperwork for intake sessions of the TCU Counseling, Testing, and Mental Health Center (CTMH).  Currently enrolled students who have previously called our Center to schedule an appointment are asked to print out the forms and complete them to bring to their scheduled intake appointment time.  Students are advised that the completed forms must be delivered in person, and cannot be delivered by fax or email, as therapist will only review forms delivered by students who already have a scheduled appointment. In addition, students are advised that completing the intake forms do not establish a therapeutic relationship with any staff member of CTMH.  Therapist-client relationships can only be established by students attending their initial counseling appointment.  Students experiencing an emergency should contact the Campus Police at 817-257-7777 or call 911.
